Actually, it does matter which direction they come from. 12-to-6 elbows specifically refers to elbows thrown straight down to the ground with the point of the elbow. You can throw the same type of elbow from your back ala Anderson Silva vs Travis Lutter.

I dont think it's physically possible to do 12-6 elbows from the bottom. What he did would be called 3-9 I guess and there's nothing illegal about that in the slightest. Mendes was complaining about them being close to the back of the head (I'm guessing)

This, unified rules say "Striking downward using the point of the elbow". Connor was throwing horizontal elbows, not really much different from a spinning elbow.
