You'll half to fill out an application for a license but they wont except* it if it is in all caps.

Each state has its own laws regarding professional and amateur MMA. Some allow one and not the other. Some require licensing and some don't, and of course, many states still don't allow the sport at all, in any form.
Most states have a state Athletic Commission. Athletic Control Board. Boxing Commision or something similarly named that regulates things there. Write or call the individual state government to find out what they allow and require.

The license is very easy to obtain, you simply have to pass a written test. Sucks to be you.

in my years of experience I have never seen an ammy have to have a license.

It is best to have your trainer or the promoter arrange this for you.

You must first register your hands as lethal weapons.

Currently in IN you do not half to have a license (have or hoe) to fight ammy mma. Unfortunately, they are getting ready to to put the Indiana boxing commission in charge of mma, and I believe ammies will then be required to be licensed.

It varies significantly state to state. Some states require amateur event
promoters to be licensed...but not the amateurs that fight for them.

Other states require amateur fighters to be 'registered' but they are not
issued a license. Still others do not regulate MMA and promoters can have
pro's fight amateurs, dudes out of the ticket line can sign up at the last
minute, and they can hand out "Hello my Name is" stickers and tell kids
they're licensed if they want.
