It's not different except for one absolutely critical detail: Unlike baseball, basketball, football, etc. MMA involves two people punching each other in the head.

So if I'm a fighter I probably should be thinking what happens if I get serious head trauma from a fight, retire and have problems the rest of my life because the guy that KOed me was juiced to the gills?

Should you only sign to fight weaklings? Do you have any reason to think you will take more serious trauma from a punch from someone who had used PEDs than another guy who is just naturally a hard hitter? Guys who get stronger through improved strength programs don't typically seem to develop noticeably more KO power; do you have a reason to think that guys who get stronger because of PEDs would?

P.S. they can't be "juiced to the gills" since that implies the stuff is still in their system at fight time.

Clean is a subjective term anyway. As we progress in tech and medicine it's only going to make the Grey area larger. The truth is they'll never be able to regulate a "clean sport".

The "punch to the head" was more or less a figure of speech.

Maybe a better way to put it is that fighters are juicing for an advantage (or else they wouldn't bother). Whether that advantage is they can train longer/harder, gain more strength/speed/power, can punch/kick harder, whatever, it is hurting their opponents - if not noticeably so physically clearly it is in their wallet via losses.

And whether the stuff is in their system or not fight time, it was when they were training so ... yes, it may have helped them hit harder (see the last paragraph).
