Depends on the age and how I would act. There is no scenario I can imagine being accepting of his mental disorder though. Let just say he is 4 then the wife and I would sit him down to talk. If he was 16 he would see Psychologist.

I would tell him that is completely abnormal behavior, show him the statisical data that shows they commit suicide at an extremely elevated rate, and find him a good therapist that doesn't subscribe to the transgender nonsense.

This is really something you need to stay ahead of. If you're at this point it's already too late for your son to have a normal life more than likely.

He's probably at least gay or has been abused.

Just gotta help him how you can with some therapy and acceptance (if he's gay, not with transgender bullshit)
