I think Shogun of Harlem retired a Navy 0-4. 

 

I doubt anyone higheer would admit it since they would have been a commanding officer somewhere.

Well the first two ranks of O-2 and O-3 are achieved by just serving in the allotted time. 

 

After that, it depends on whether or not you qualify for quotas, your education, performance, how much the command will or will not attribute that performance to you etc.
