As mentioned in my comment above was Stan Hansen had terrible eyesight and was a really stiff worker. He potatoed Vader at one point in the match and his eyeball popped out of his socket. He put it back in and kept on with the match.

Oh damn. I heard the story the way I told it from a worker pal of mine. Never really questioned it.
