I think this resulted from a BIG misunderstanding.
The promoters backstage called for the ref to tell Ishii to start fading. Fedor didn't hear the ref and figured he would keep fighting and threw a hard punch that Ishii was supposed to block as per their rehearsal before the fight.
Ishii didn't block the punch and the real results came through. Same kind of things can happen in pro-wrestling when communication isn't 100%

I still think it was a work and just because a guy got a fluke injury does not mean it wasn't a fix!
I am going to quote a tiny excerpt from fellow poster Brigham: "breaks Ishii's brain". That is basically in laymans terms what happened, that doesnt sound like a fluke to me. Fedor hit him so hard that his brain swole up to abnormal size. If the fight was a "work" then a fluke injury would have been a broken nose or some knocked out teeth.

This is shocking. I had it on good word from the UG that the fight was a work. Oh well, hope he recovers fully.

One a side note: All this talk about shots to the back of the head, and soccer kicks and stomps being so horribly dangerous and a simple punch to the face caused and elite athlete to suffer very serious brain trauma...
Pretty sure it wasn't a single punch that did this. He caught Ishii with a ton of good shots throughout the fight.
